grant Omnigon Pty Ltd ACN 066 753 875 (“Omnigon”) and its supplying companies (including T.G. Eakin Limited Company No NI009807, and Welland Medical Ltd Company No 02170535 (collectively “Supplying Companies”) an irrevocable, royalty-free, non-exclusive, worldwide perpetual license (“Licence”) for their internal and external use, replication and distribution of the provided photos and written/spoken content (including but not limited to stories, case studies, testimonials and email feedback) in full or in part (“Materials”) without further compensation, notification or permission. Use of the Materials would be for educational and promotional purposes including (but not limited to) printed materials such as brochures, publications, display materials, adverts, or digital materials including audiovisual, websites, social media applications, mobile applications and other digital content or media that may be required. I acknowledge and agree that I will not seek any royalties, commissions or similar payment or fees for Omnigon and the Supplying Companies’ use of the Materials, and I agree to hold Omnigon and the Supplying Companies’ harmless from all and any liability which may arise from their use of the Materials. I represent and warrant that I am the owner of the copyright in the Materials or that I have permission from the copyright owner necessary to grant the License. I consent to the collection, use and disclosure of my personal information by Omnigon and the Supplying Companies to enable the effective use of the Material in accordance with the License. I agree for Omnigon and the Supplying Companies to use this information in any media for future promotional, marketing and publicity purposes without any further reference, payment or other compensation.
